Waiver, Release and Indemnification Form

1. Paddling outrigger canoe is a considerable test of an individuals personal fitness, strength, coordination and endurance. Participation in this sport poses a very real risk of injury to each and every participant and other non-participants nearing the activity. These risks include, but are not limited to the following: weather, equipment failure, drowning, hypothermia, boat collision, and injury caused by other participants. In consideration of volunteering to participate in the canoe race the undersigned hereby agrees to abide by the rules and regulations established by the Jericho Outrigger Canoe Club and it's directors.

2. For myself, my executors, administrators, next of kin, successors and assignors I hereby: waive and release any and all claims that I may have against Jericho Outrigger Canoe Club, Jericho Sailing Centre their respective officers, directors, members, volunteers, race committee members, agents, sponsors, or assigns (the"release") including any and all claims for damage caused by any of them arising out of my participation in the paddling race, or related events together with any costs including lawyers fees that may be incurred as a result of any such claims whether valid or not.

3. I hereby acknowledge that I have sole responsibility for my personal possessions and athletic equipment during the paddling activities, races and related activities.

4. I hereby acknowledge the participation in paddling competition is potentially hazardous. Therefore I hereby release the CANADIAN OUTRIGGER RACING ASSOCIATION (CORA), it's directors, officers, members, volunteers, sponsors, of any liability resulting from injury from death during events sponsored or sanctioned by CORA.

5. I hereby attest that I am physically fit and have sufficiently trained for competition.
